USMLE Step 1 Writing Guide

Writing Overview

The USMLE Step 1 tests basic science knowledge essential to medicine. Since January 2022 it reports pass/fail only — no numerical score. It's an 8-hour computer-based test with up to 280 multiple-choice items covering anatomy, physiology, biochemistry, microbiology, pharmacology, pathology, and behavioral sciences. Usually taken after the second year of medical school.

Note: USMLE Step 1 does not have a dedicated writing section. This guide covers general test preparation.

High-Score Strategies

  • 1.Start with a dedicated Qbank like UWorld — it's the gold standard.
  • 2.Read First Aid 2–3 times, annotating with Qbank insights.
  • 3.Do practice questions daily, even during coursework.
  • 4.Focus on high-yield topics: pathology, pharmacology, physiology, microbiology.
  • 5.Take NBME practice exams every 1–2 weeks in dedicated prep.
  • 6.Sleep 7–8 hours — memory consolidation matters more than cramming.
